PostPartum Haemorrhage (PPH)

I would consider myself lucky to be on-call on 2 nights in a week whereby 2 cases of PPH occurred requiring surgery. The best part is that I don't have to be the House Officer running back and forth to the blood bank, instead I was inside the OT to witness:

1. Total hysterectomy
- Para 4

2. B-Lynch suture
- Para 1 (with triplet pregnancy)

I have always been reading up and presenting about B-Lynch suture when I was a medical student. Now, after witnessing it with my own eyes, I think it is so cool! ;)



For House Officers out there, don't forget what you have to do in an emergency of PPH in the ward:
- ABC
- call for help (red alert)
- 2 large bore IV line & blood investigations: FBC, U&E, LFT, Coag screen, group x-match)
- crystalloid infusion
- monitor vital signs

Management of PPH
Uterine massage
Medical
→ 500mcg ergometrine IV/IM
→ Oxytocin infusion (40IU) → 10IU IV
→ Carboprost 250mcg IM (up to 4 doses)
→ misoprostol 800-1000 mcg PR
Bimanual compression
Surgical
Repair genital tract injuries
Uterine temponade (Rusch balloon)
Laparatomy
· B-Lynch suture
· Internal iliac/uterine A ligation → hysterectomy (50%)
· Uterine A embolization
· Total/subtotal hysterectomy

Prevention:
Active management of 3rd stage (AMTS) – syntometrine, CTT, uterine massage

1st posting: OBS & GYN

As you know, I started work in PPUKM on the 1st of November (yeah, they've changed from H to PP). I am currently in O&G department.

I only did my tagging (where we have to learn stuff quick from another Houseman) in just a week, and started on-call on my own on my 8th day of "life" in that graveyard. So it was a freaking nightmare.. I was on-call every other day (EOD), which means (for non-medical readers) start work 7am today and come home at 5pm tomorrow and the cycle continues every day. I was off tag after day 7 and the 2nd week, I was on-call EOD again. (T_T)

Work has it's ups and downs (life in general is like that right?). Some days are fine.. not that many patients, a few discharges and no ward rounds with the Prof. But most of the days are hell, so many patients delivering at the same time, patient in high-dependency unit started to deteriorate, referrals and so many clerical things to do, and suddenly was called to assist C-section.. ALL AT ONCE!

And yes, to answer the burning questions in your mind, I do conduct deliveries, and not just put my fingers in ppl's vajayjays but cut & sew them as well.

Mum started complaining of how little time she got to see me at home, because most of the time I'll be sleeping in my room every chance I got, even in the car (when I'm not driving that is).

My friend, Aziah is in the same hospital but I've never met her in the hospital, NOT EVEN ONCE!!! Oh wait, I think I did in the first week, just her silhouette though... *sigh*

Okay.. okay.. fine... I do learn loadsss of things these past 3 weeks, plus I lost 2kg already.. So there is an up side to all of these... ;)

Work starts tomorrow! For real!!!

Earlier this week, I had a 5 days induction course here in the heart of Kuala Lumpur city. I was in Dynasty Hotel, Jalan Ipoh which is like 10-15 minutes drive away from my house (given that there is no traffic jam, of course).

It was an enjoyable experience, with a great roommate and making friends with other doctors. Everything else was great, even the food. Except for the cleanliness of the hotel. It does not live up to the standard of 4 star hotel at all. They need a major refurbishing and pest control operation from top to bottom.

Enough about the hotel. So during the course, everyone was informed of the next step in our life. Our postings to which hospital for the next 2 years. I got PPUKM in Cheras! (my 2nd choice) Yeayhh! In another word, I'LL BE STARTING WORK TOMORROW!!!! Am I ready for this??? T_T

Expect no updates at least for the next 14 days.. Tagging will start really early in the morning and finishes really late at night. Or maybe I'll be updating more than usual.. who knows.. heh.

Sorry this has to be in Malay because I am damn angry

Bank Simpanan Nasional = BSN = Bodoh Sangat Nakmampos! (okay, takde M tp nk jugak tambah!)

Aku pergi BSN td dgn kakak aku, hajatnye nk active kan blk account aku yg dh 5 tahun tak jenguk. (sbb aku takde dlm negara ni). Kalau ikot rules & regulation kat buku akaun tuh, 7 tahun tak aktif baru la dia tutup akaun. So, previous years aku blk Msia pun takde la aku kesah nk gi aktifkn account tu sbb dah t'tulis mcm tu kan..

But td aku pegi nk msk kn duit, dia kata account dah tutup!!! Aku pun mcm WTH la kan, account anak buah aku pun 5 tahun tak aktif, msk rm50 semlm ok jee... Bila aku tanye, si minah kat kaunter ni yg lebih baik takyah kerje yg kene jumpe customer sbb takde budi pekerti yg Melayu banggakan sgt... dia kata 2 tahun je tak aktif account tutup. Dari WTH aku pun tukar WTF la kan.. (dalam hati je la masa tu... naseb baek kakak aku ada)

2-3 tahun lepas, kakak aku ada masukkan check. Aku ada dapat check dr sebuah institusi yg aku penah blaja sebelum fly dulu sbb aku jd facilitator masa minggu haluansiswa. Kira gaji aku la utk seminggu tu.. So, kalau nk ikotkan, takde la akaun aku tu tak aktif sampai 7 tahun kan?? Yg bestnye, minah kat kaunter td tu ckp, "Balance rm24 je, bukak akaun baru je dik..." (based on balance dlm buku yg aku tak update 5 thn)... Eh, hello! Abes check aku tu kau nk byr ke?? Tak kuasa aku nk bukak account kat situ lg.. 7 generasi aku pun tak akan lah!! Mati hidup semula pun jgn mimpi!!

Dia kata sbb aku pakai atm card dulu, tolak rm8 per year. Eh, cukup la balance aku kalau nk potong pun. Akaun CIMB aku pun tak aktif 5 tahun ari tu, aku blk last year masuk duit kluar blk terus aktif balik!! Apa barang?? CIMB pun aku pakai kad. Mmg potong rm8 per year, tp pada tahun yg aku gune je!! Tahun aku tak guna takde la charge jugak suka hati mak bapak dia.

Mmg la, balance aku dlm akaun tu takde la beribu berjuta. Tp surat ke ape ke takde pun sampai kat rumah, on printed 7 tahun, tetibe ckp 2 tahun. Aku rasa kalau aku buat court case ni bole menang! Bukan lah aku kesah sgt duit dlm bank tu, kalau nk burn pun burn la.. Bole je hidup takde duit dlm tu. Tapi jgn la ckp, "Dijamin oleh Kerajaan" tp tak ikot procedure langsung kan...

Lagi satu, pasal banking kat Malaysia.. sume bende dia nk charge. ATM card 8 hinggit setahun la, siap ada limit brape kali sebulan plak tu bole pakai. Pastu kalau nk guna ATM kene kat specific ATM bank tu je pulak, charge extra kalau guna atm lain dan macam2 lg..
Masa aku kat sana, bayar duty stamp je every year, pastu lantak la nk guna ATM card tu, xde yearly charge pastu suka hati nk cucuk atm mane FOC ok!

Solution to my dilemma, BB Torch it is..

If you've been reading my blog, you knew the dilemma I'm having.. iPhone4 vs Blackberry. For me it's more than just a gadget. Choosing has other implications as well: College friends vs School friends, Facetime vs BBM.

1. You started thinking about loans and debts and buying cars and buying properties, and these kinda things are the topics of discussions when hanging out with friends.

2. You started to think of how to spend your money wisely, investments and you actually understand bankers' language, and understand your taxes.

3. You started planning to get married, not just thinking about it, but really started buying stuff and have a date in mind.

4. You started planning at least 2 years ahead, if not 10 years ahead.

5. You started to have Plan A and Plan B... and maybe Plan C.. in planning your career.

6. You started to hate it when you hear the "latest hits" on the radio.. and you started searching for "oldies".. 90s songs are considered oldies now okaaayyyy.. ??? *sigh*

7. You started to hate what you see on TV, you think teenagers are ridiculous with their fashion and music senses. Not mentioning the hair, make-up and the PDA.

8. And of course the physical signs, you started having gray hair and wrinkles you swear you never noticed before and buying unnecessary creams for eye-bags and cellulite.

We celebrated Miza's birthday last weekend, as she's studying in Johor. Our aunt brought us to Tony Roma's for her b'day dinner. I had the Chicken Pesto Salad which was to die for!!

Then we head to Starbucks, of course.
To burn all the calories off, we then had countless rounds of PS Rock band~! I love playing the drum, it really tests your coordination! And I got this singing one of the Beatles' hit (can't remember which) *woot woot*

Wedding bells~~

I have sooo many things to share with you but.. but.. but..
Ermmm.. nope. I don't have any excuses. :P.. I'm not busy at all.. I have soo much free time it's killing me.. heh.

Apart from sending and picking up the kids to and from school, grocery shopping with my sisters, playing with my cats and lists of television series to stream online, I basically have nothing to do at home.. My friends are all working during week days, my boyfriend is busy busy busy... and the ministry of health are saying that they don't have enough doctors but are so freaking slow to send me my induction letter. pppffttt... I know, I'm complaining I'm bored to death now, but I will be complaining of how I wish for a holiday once I started working.. Homo-sapiens are never content with their lives. heh.

Let's see.. where shall we start...
I don't know when this trend of open houses and weddings in every corner during the month of Syawal started. As I remembered, spending Syawal in Malaysia (6-7 years ago), the invitations are not as many and the traffic was not as bad.. Really made u think twice to drive anywhere at all...

So I am taking this opportunity to wish the newlyweds CONGRATULATIONS on their weddings...

Darling Gina~~

We sat next to each other in class when we were 11 years old.
We were then separated when I moved away.
But we stay in-touch until now (thanx to snail mail, then email, then Friendster and now Facebook).
Even when she and I were in Southern & Northern Hemispheres, thousands of miles apart.
Occasional get-together and lunches kept us updated (though I wish for more of these sessions).

Yesterday was her birthday & her engagement day.. So, wishing you..

Reminiscing....


I miss cooking and break-fast with them.
I miss the sadness and laughter we shared spending Eid away from our families.
I miss the arguments of who would lead the Tarawikh every night during Ramadhan.
I miss all the dishes we prepared ourselves on first day of Syawal, even when we have classes.
I miss our open-house and feasts as well as hours of photography sessions just to get few good shots to be posted on FB.
They are my sisters, they are my family.. I miss you girls!! and boys.. hehe. ;)

Venting out.. long overdue...

Some people are just plain selfish that sometimes makes u wanna bang your head on the wall and scream your lungs out.. (thank God it's Ramadhan and thank God we have blogs nowadays to let our steam off).

These kinda people always leave you with problems and headaches. I don't know whether they just purposely do things that will only satisfy them and only them without thinking the consequences to the people around them. It's probably pure ignorance or simply S.E.L.F.I.S.H.

Eh hello! Please la think one step ahead. Plan things out properly. If you expect people to be nice to you, be nice to them. Learn common courtesies, like 'thank you' and 'sorry'. These things are easy to do. My little gems also know how to say thank you and say please. Which planet are you living in huh?

p/s: this is not just about one person, this is about 2 people.. heh.

Good news, Sad news

I passed my driving test today!!! weehooo~~!! FINALLY!!! After delaying it for 8 yrs, now I'll be driving around for the next 2 yrs with this sticker on my car..



(err... what car?). I mean my Mom's car or anybody else's who is not afraid to let me drive theirs... =P. I'll buy my own car soon... ;)

On a different note, my cat, Nottie is no longer with us.. *sobs*. She was run over by my sister when she was backing up the Merc from the driveway. We always check around the car to make sure Nottie wasn't around, and if she is we would lock her up in her cage. But today she was nowhere to be found before my sister back up the car. She suddenly came running out of nowhere and her life ended then.
In a way I am glad that I wasn't at home when it happened as I would have cried my eyes out witnessing such a tragedy. I have always been the one to feed her and play with her and hug her and bathe her..
I will always remember you and love u, Nottie~~ Rest In Peace...

My Ramadhan Routine

If you've been wondering what the heck am I doing during my free time now that I've finished my degree but still haven't started working, allow me to tell you..

Most of the days, early in the morning, I would accompany my sister to the market (Pasar Dato' Keramat) to get her supplies of fresh meat as well as some groceries for break-fast and dinner. And now, I'm starting to learn my fishes and what type of cooking is suitable for each fish. I told you, I am a domestic goddess in the making. heh.

When we got home, my sister would start cooking her Daging Cincang Asam Pedas which will be sold in Bazaar Ramadhan.
Yummy~!!

Myself, on the other hand, would help out my neighbour for his Pucuk Ubi Tempoyak for Bazaar as well. Help out as in volunteer work, and in the West, you still got paid for volunteering, so I will get my pay at the end of the month. =P

After I've worked my biceps (right arm mainly), chopping and slicing, I would then help my sister out with her cookies, thus building my right biceps some more, rolling the dough.

After prayers and some rest, some days we might go to super markets to get some groceries, and pick up the kids from school. Got home and help out my sister in the kitchen for our break-fast and dinner. Since this Ramadhan, I've only been to the bazaar once! (when I thought that was the thing I miss about fasting in Malaysia).
And I've been to the surau once for tarawikh as I am too tired to leave the house. I would do it on my own at home if I do not doze off first.

That's basically how I spend my day this Ramadhan.

So, if you're looking for Kampung style food for buka puasa (Daging cincang asam pedas, pucuk ubi tempoyak with or without petai, ikan patin masak tempoyak and daging salai lemak cili padi), you will find the stall in Taman Melawati's Bazaar Ramdhan. You won't miss it, just look for this periuk belanga. ;)

Hiking @ Broga Hill

Last Sunday was a fun-filled day which started really early, at 4 in the morning! Me, Mr Boyfie and 5 friends from high-school went hiking. We were hoping to catch the sunrise at the peak, but the number of people went hiking on the same time on the same day was unbelievable, it was like having Kuala Lumpur's traffic jam on the steep hill, like seriously.. Exactly like MRR2 on a Friday afternoon, I'm telling you..
The climb up was great, just a few places with slippery slopes. But the journey down was not so bad, for me at least, because I fell down flat on my bum.. *sobs*. Blame the slippery slopes and my 3 year old Nike sneakers. =P

After we were back on flat-land, we went for breakfast and had a loooonggg discussion to continue our adventurous day.. So we decided on going to Kemensah river for a swim. Well, just dipping our feet for some of us. *wink*
The day continued on a lunch at Kampung Thai in Ampang.I still haven't found the best Pad Thai in Malaysia.. any suggestion people?

All in all, I had an awesome time with my friends laughing our heads off and updating on gossips. I just can't believe it has really been 8 years since I left school.. Daym, I'm old..

Kids.. =)

Remember Bill Cosby's 'Kids say the darndest things'? When you have 13 nieces & nephews, you can't help it but laugh at the silly things they say and do..

Example 1
My 1 1/2 year old nephew was slurping too much Chocolate Ice-blended only to stop and said, "Atit" (translated: It hurts!) and holding his head... He was having brain freeze! LOL

Example 2
My 2 1/2 year old nephew keeps calling my boyfriend "Abang" (translated: brother) instead of Uncle.. I keep on correcting him and he would refused to correct himself accordingly.. So I purposely refer to my boyfriend as "Abang" and he was confused and started calling him Uncle instead~! *wink*

Example 3
When my 7 year old niece was younger (around the age of 5), she refused to be straight-forward in saying things. For instance, she would say she's having stomach-ache instead of having to go to the loo. So, every time she said she's having stomach-ache, that's the cue she needed to go.. And when she's sleepy, she would say her eyes are getting smaller (in Malay: "mata adik dah kecik!") instead of wanting to go to bed! =D

These are just some examples.. These kinda things make you adore them no matter how naughty they are... =)

My nephew and my mum had recently brought home 3 kittens and yesterday I had the honour of bathing them! It was such an honour as these 3 little fur-balls are the most "manja" kittens I've ever known. They didn't even purred when I splashed cold water on their "comot" faces. hehe.

Domestic Goddess in the making

2 nights ago, I cooked dinner for the whole family. I made roast chicken with cranberry sauce and potato gratin for the main course. Thanx to my dear friend Kamas and her hubby who taught me Jamie's style!

Dad's B'day, Blackberry & Love triangle

Tuesday was Daddy's b'day.. We had home-made steamboat for dinner and Boston Cream Pie for Dad's b'day cake (Yummy~!)

For his presents, I bought him a frame, nails & sandwich bag (things he needed) which were wrapped nicely. He didn't know that we had bought him a Blackberry for the REAL present.. *wink*

Blackberry isn't as good as the hype really.. It refused to switch on even with the battery fully charged. It was fine this morning (I even had the chance to play with the new gadget for a while) but it went kaput. Thank God for warranties..

Earlier that day, I had a date with Mr. Boyfie. We went for a movie in TGV WangsaWalk. My 2nd time there, first was with my cousin for Karate Kid. 2nd time around was of course, for Eclipse.. I know it is quite late to watch it 5 days after the premier given that I am reading the books for the third time now. But I'm trying to avoid the crowds.. and to avoid being labeled as Twi-hards. And yeah, the love triangle in the books are less cheesy than the movies.
But the coolest part was of course the fighting scenes, especially when it involves the Cullens.

We voted, the best couple is... *drum rolls*

World Cup...??

I am never really into football.. and thank God my boyfriend is not really into it either.. ;)
But somehow I am glad I stayed up last night to watch Espana kicked Deutschland's ass off the cup.. woohooo~~!!
When I was in Cork, 2 girls in the house were really into football especially Kakya.. she would scream her lungs off everytime she watches a match. And everytime I joined them (usually only the final match of a league/cup), I would support the opposite team (just for the fun of it) as well for the fact that the team that I usually like would lose, so the girls are usually delighted.

So for this World Cup final, between Netherlands and Spain... I would definitely stay up again to watch the match.. Supporting, of course.. ESPANA~!! =D

p/s: sorry if my bad-luck would cost Espana the title, why not wait for Paul the Octopus...LOL

10th day in Malaysia.. weee~~!!

I'm home!!! for good!! yeayyhh!! =P
So, we arrived in KLIA on the 25th of June.. My sisters, brother & the kids were there..

and of course, mr. boyfie was there too! ;). and I was knackered, smelly and with blocked-nose. *sniffs*

The next evening, we went to RedboxRia in Sogo for my welcome home party as well as birthday celebration for my niece, my nephew and my sister.

It was the 1st time since i-can't-remember-when, all 7 siblings were gathered under one roof.

And then there were hanging out sessions with cousins and Wii at their brand-new home in Sering Ukay. (we should have taken pictures, adik!)

And then there were times I just sat at home, with tissue papers surrounding me on the bed and Charlaine Harris accompanying me.

Yesterday was a very interesting day indeed.. Spent the whole day at Mr. Boyfie's parents', *coughs* my future in-laws' *coughs*(InsyaAllah). Had lunch there and brought them home-baked scones (Mr. boyfie's favourite!) and played bowling @ Alamanda with the whole clan afterwards. ;)


Oh! And I'm in the process of getting my driving licence! Finally!! =P
